A Health Home is a group of organizations working closely together to provide physical health, mental health, and addiction services. The Health Home will assign a Care Manager who brings together all of these services for an individual.
Home and Community-Based Services are available to people who are enrolled in a Health and Recovery Plan (HARP). Those enrolled in HARP are eligible for an enhanced package of benefits called Home and Community-Based Services.
The Single Point of Access (SPOA) program helps providers connect people with serious mental illness to mental health services that can connect them to treatment, communicate with providers, and get help finding benefits.
